The title began in 1957 and lasted first 1980 when it was first abandoned when Jerry Jarrett took over the Mid-American titles from Nick Gulas. Jarrett revived it in 1981, making it a part of the Memphis-based Continental Wrestling Association, and it then lasted until 1987 when it was unified with the newly created CWA (now renamed Championship Wrestling Association) Heavyweight Championship.

Shawn Shultz won The NWA Mid-American Championship tournament held in Versailles Indiana on October 17th 2015 to become the new NWA Mid-America Champion. Johnny Justice defeated Shawn Shultz on January 29th, in Seymour, Indiana by submission. Johnny Justice lost the title to his younger brother when he was pinned in Madison Indiana by Dino Stephanapolis on March 12th 2016 only to lose it one week later in a rematch. On April 9th, 2016 in Madison Indiana "King" Nicolas Noble defeated Johnny Justice for the NWA Mid-America Championship after The Blackheart struck Johnny Justice with the belt. Johnny Justice defeated Nic Noble by submission on April 22, 2016 at The Clash of Champions event in Hanover Indiana, but lost it one night later to Eric Draven (Professional Wrestler).

April 30th, 2016 Johnny Justice pinned Eric Draven in controversial fashion to begin his 4th reign as NWA Mid-America Champion. May 6th Eric Draven defeated Johnny Justice, Dino Stephanapolis, and Nic Noble in a fatal four way match to become the Mid-America Champion. On June 10th in Hanover Indiana Eric Draven faked an in-ring leg injury to distract the referee while partner Josh Lewis struck Johnny Justice with the Mid-America Championship. Draven then made the pin ending Johnny's 5th reign as champion and beginning Draven's 3rd.

On September 24th in Madison Indiana Josh Lewis defeated Johnny Justice to win the NWA Mid-America Championship becoming the first person in history to hold The NWA Supreme, NWA Supreme United States, and NWA Mid-America Championships all at the same time. On November 5th, King Nicolas Noble began his second reign as champion by defeating Josh Lewis (professional wrestler).

On February 24, 2017 Eric Draven pinned Johnny Justice in a triple threat (also involving Josh Lewis) at a WCWO in Indianapolis Indiana to become The NWA Mid-America Champion. Though the match did not take place on an NWA event all three wrestlers were NWA wrestlers and this match was sanctioned by The NWA.

On March 18th, 2017 Van Martigan defeated Eric Draven using a sleeper hold, referee Doug Morrell called for the bell after Draven's hand dropped the third time at 29:59 into the 30 minute time limit match. Winning this match Van Martigan became the lightest wrestler in history to hold this prestigious championship.
