De Carlos was elected president of Real Madrid after the death of Santiago Bernabéu. During his reign the club won two Spanish Championships, two Spanish Cups, one (now defunct) League Cup, and one UEFA Cup, but failed to win its seventh European Cup after losing 0-1 to Liverpool in the 1981 final and its first UEFA Cup Winners' Cup following their 1983 defeat to Aberdeen in the final.

He did not run in the elections of 1985 and was succeeded by Ramón Mendoza at the helm of the club.
