KY 459 begins at an intersection with KY 6 south-southwest of Dishman Springs, within the southwestern part of Knox County. It travels to the southwest and crosses over Big Indian Creek and begins paralleling it. The highway curves to the southeast and crosses over the creek again. A short distance later, it bends slightly to the south-southeast and leaves the creek. It curves to the east-southeast and begins paralleling the Cumberland River. It curves to the northeast and passes Dr. Thomas Walker State Park. It curves to the north-northwest and passes Warfield Cemetery. KY 459 curves to the east-northeast and passes Powell Cemetery. The highway curves to the east-southeast and passes Mayhew Cemetery just before intersecting the western terminus of KY 2423 (Sharp Gap Road). It curves to the south-southeast and enters Barbourville. It curves to the northeast, leaving the Cumberland River. It curves to the east-northeast and crosses over Richland Creek. Then, the highway turns left onto Sycamore Drive. It travels to the north-northwest for one block before it turns right and resumes its east-northeastward trek for one block. At that point, it meets its eastern terminus, an intersection with KY 11 (South Main Street / Daniel Boone Drive).
