Foothill Community Health Center is a federally qualified health center operating in the County of Santa Clara. An FQHC follows the requirements of the U.S. Department of Health and Human Services, Health Resources and Services Administration. The mission of an FQHC is to provide healthcare services to the uninsured and under-insured.

History

The history of Foothill Community Health Center dates back to 1984 as a private physician’s office. In 1996, the physicians saw a need to serve the community as a nonprofit health clinic and the San Jose Foothill Family Community Clinic began serving low-income families and individuals in East San Jose. The clinic provided comprehensive-primary medical care to the community from its Story Road Office in San Jose, California. In 2006, the San Jose Foothill Family Community Clinic received recognition as a federally qualified health center, as defined by the U.S. Department of Health and Human Services, Health Resources and Services Administration. As an FQHC, community health clinics are eligible to receive funding assistance from the U.S. Department of Health and Human Services, Health Resources and Services Administration. The San Jose Foothill Family Community Clinic expanded its medical care services under new executive leadership in 2010. During 2015, Foothill Community Health Center (new business name) expanded its primary healthcare services to include: medical, dental, vision, pediatrics, OB/GYN, podiatry, health & nutrition education, and behavior healthcare services through six school-based health clinics and six neighborhood health centers.

Other

On the third Saturday of every month, Foothill Community Health Center distributes free produce to the community. During the Holiday Season, Foothill provides gifts to children aged zero to eighteen. Each month, Foothill hosts health screenings at locations where its community shops, worships, and congregate. During 2015, Foothill hosted its first Annual Women’s Health Conference. In addition, Foothill Community Health Center has a pilot program to create systematic changes in the development of low-income at-risk young children who lack resources that are generally available to middle/higher income families. Foothill’s Student Health & Wellness Program provides physical and mental health services to children beginning in elementary school and ending in high school. The Student Health & Wellness Program seeks to determine the correlation between greater student academic success with a student's health and wellness.
