The route begins at Highway 46 at Dogwood near Jenkins' Ferry State Park and runs north. The highway intersects Highway 190 and continues north to Prattsville. In the city, Highway 291 has a rare officially designated exception overlap with US 270 for 0.06 miles (0.097 km). The highway continues north to enter Tull, ending at the southern terminus of Highway 190.
