Born in Dos Torres, Province of Córdoba, Medrán played youth football for local teams before joining Real Madrid's youth setup in 2011, aged 17.

After being promoted to the C-team in the 2013 summer, Medrán played his first match as a professional on 23 February 2014, coming on as a second-half substitute in a 2–0 win at Real Zaragoza for the B-team in the Segunda División championship. He scored his first professional goals on 31 May, netting a brace in a 2–1 home success against CE Sabadell FC.

Medrán made his first team – and La Liga – debut on 18 October 2014, replacing Luka Modrić in the 79th minute of a 5–0 away routing over Levante UD. On 9 December 2014, in his UEFA Champions League debut, he scored his first senior goal for the club, after replacing Gareth Bale in the 83rd minute and netting the last of a 4–0 win over Ludogorets Razgrad five minutes later.
