Multiple ranged weapons from handguns to crossbows are at Alex's disposal. The game will feature quick time events similar to Heavy Rain, a chopper chase, and allow the player to ride a police motorbike. Stereoscopic 3D will be supported.

The story follows a 20-year-old woman and California college student named Alex across the world, from the misty shores of San Francisco to the fearsome and foreboding corners of the Old Continent. There Alex learns that assumptions can be lethally deceptive and that the seemingly safe modern world is but half of a chamber divided by a dark curtain... that beyond that opaque veil lies the embodiment of nightmares spawned into flesh.

Alone, surrounded, and trapped in darkness Alex is forced to face the Moroi, modern vampires. The developers have stated that they hope to revive the survival horror genre by bringing back story and atmosphere. Sacrilegium will run on the Graphics Rendition and Creation Engine (GRACE) 2, a game engine developed by Reality Pump Studios. TopWare claims the improvements of GRACE 2 over GRACE on which Two Worlds ran will leave players “awestruck.” “It pleased us that we succeeded [in developing] the GRACE 2 engine in a way that enables us to present models and landscapes that make photos look like charcoal drawings compared to the game.” said Miroslaw Dymek, CEO of Reality Pump Studios. “After all the years that we‘ve dealt with the appearance of medieval stories, it was time to pool the experience acquired and integrate that into a modern action adventure. We’ve definitely achieved that with Sacrilegium.”

The game will make use of motion-capture for body movement and facial expressions, acted out by professionals and martial arts experts, directed separately by film industry virtuosos. Every character and enemy has been given a unique and separately designed set of animations.

In 2014 Topware Interactive explained, via Facebook, that Sacrilegium had been put on hold while Reality Pump focused development on finishing Raven’s Cry after the original developer, Octane Games failed to meet deadlines.
