Life Care Centers of America is the largest privately held long-term elderly care company in the US, with facilities across 28 states, and the third largest in the US. It is headquartered in Cleveland, Tennessee.
It was founded by Forrest Preston in 1970, and he remains the sole owner, chairman and CEO.
As of 2015, Life Care Centers has been the subject of a seven-year probe and federal lawsuit alleging Medicare fraud.
As of 2015, the company employs 42,000 people.
