Epipagis forsythae, Forsyth's epipagis moth, is a moth in the Crambidae family. It is found in North America, where it has been recorded from Florida.

The wingspan is about 21 mm. The forewings are brown with white markings in the antemedial, median and subterminal areas. There is a dark postmedial line which widens into a broad base at the inner margin. There is also a small white patch beyond the postmedial line at the anal angle. The hindwings are mostly white with two or three dark discal spots, as well as a dark shade in the triangular area across the apex.
