Dichomeris formulata is a moth in the Gelechiidae family. It was described by Meyrick in 1922. It is found in Brazil (Amazonas).

The wingspan is about 18 mm. The extreme costal edge of the forewings is ochreous-whitish from one-fourth to two-thirds and there is a blackish dot on the base of the costa. The stigmata is small, dark fuscous, with adjacent whitish dots posteriorly, the plical beneath the first discal. The hindwings are dark fuscous.
