Cervical degenerative changes and grading is the classification of changes to the cervix based on findings on a lateral cervical spine radiograph. The key parameters are anteophyte osteophyte formation, intervertebral disc height narrowing, and vertebral end-plate sclerosis.

Grades

Grades indicate severity and range from 0 to 4.

  • Normal (0): No degenerative changes are apparent.
  • Minimal/early (1): Radiographs display minimal anterior osteophyte formation, no height reduction and no sclerosis.
  • Mild (2): Radiographs display definite osteophytes, subtle or no reduction in disc height (<25%) and just recognizable sclerosis.
  • Moderate (3): Radiographs demonstrate definite osteophytes, moderate disc space narrowing (25-75%) and definite sclerosis including osteophyte sclerosis.
  • Gross (4): Large and multiple large osteophytes, severe narrowing of the disc space (>75%) and sclerosis with apparent irregularities.
